Last updated: 4 November 2026, for Patient Diary 1.1 on iPhone and Android
Overview
Patient Diary: Health Record ("the app") is developed and published by Patient Thread Ltd (company number 17167409), a subsidiary of Lawrence Medical Limited, which holds the ICO registration. Patient Thread Ltd is the data controller for the app.
The short version: your health data stays on your phone. Patient Thread Ltd has no server for this app. We never see, collect, sell or share your records.
The app is a personal health record. It stores what you enter so you can review it and share a PDF with your care team. It does not give medical advice.
What the app records
Only what you enter yourself, or choose to import:
- Blood pressure, pulse, blood sugar and insulin doses, weight, oxygen saturation and peak flow
- Medications, doses and reminder times
- Fluid intake and output, and symptoms with severity scores
- Food and drink, with reference nutrient values and estimated portion sizes
- Alcoholic drinks, with volume, strength and units
- Exercise sessions, including routes recorded by the in-app tracker and workouts you import from Apple Health or Health Connect
- Mindfulness, meditation, yoga and reading sessions, with optional mood scores
- Appointments and general reminders
Where your data is stored
Everything is stored on your phone, in the app's own private storage. Nothing is sent to Patient Thread Ltd or to any server we run. We cannot see, recover or delete your records.
iPhone. If your iPhone backs up to iCloud, the app's data is included in that device backup under your own Apple account. That backup is governed by Apple's privacy policy, and Patient Thread Ltd has no access to it.
Android. If Android backup is switched on for your phone, the app's data may be included in your Google account backup. That backup is governed by Google's privacy policy, and Patient Thread Ltd has no access to it.
Location
The exercise tracker uses your phone's location only while a tracked session is running, to measure distance and pace and to draw your route. Tracking continues while the app is in the background during that session, and on Android a notification is shown while it records. When you stop the session, location use stops.
The route is saved with that session on your phone. Location is not collected at any other time and is never sent to Patient Thread Ltd. You can decline location access and still log exercise by hand.
Apple Health and Health Connect
If you choose "Import from Health", the app reads your recent workouts so that sessions recorded on a watch or another app appear in your exercise log. On iPhone this reads workouts and the heart rate recorded during those workouts from Apple Health. On Android this reads exercise sessions from Health Connect. In both cases the app looks back 30 days and copies the sessions into your exercise log on your phone.
The app never writes to Apple Health or Health Connect. Health data is never sent to Patient Thread Ltd or to any third party, is never used for advertising, and is used only to populate your exercise log. You can withdraw access at any time in the Health app (Sharing, Apps) on iPhone, or in Health Connect on Android.
Food data
Nutrient values come from McCance and Widdowson's Composition of Foods Integrated Dataset (CoFID) 2021, published by Public Health England and used under the Open Government Licence v3.0. The table is bundled inside the app and searches run on your phone. Nothing you type is sent to any server. Portion sizes are estimates added by Patient Thread Ltd and are not part of CoFID. Nutrient figures are reference values for typical foods and are a guide, not a measurement.
Maps
Routes are drawn on a map. On iPhone the map is Apple Maps, which fetches map imagery from Apple under Apple's privacy policy. On Android the map imagery comes from OpenStreetMap. To draw the map, your phone requests map tiles for the area shown, which means the OpenStreetMap Foundation receives your phone's internet address and the map area requested. Your route and your health records are not sent. The map is only loaded when you open the tracker or a session that has a route.
Notifications
Medication reminders, general reminders and the mindfulness timer use local notifications scheduled on your phone. No push notification service is used and no notification content leaves your phone.
On iPhone, critical alert notifications for time-sensitive medications require your explicit permission and are processed on your phone only.
Analytics, tracking and third parties
The app contains no analytics, no advertising and no tracking software. The only network requests it makes are the map tile requests described above and links you choose to open in your browser, for example NHS alcohol advice.
Sharing
The PDF export is created on your phone. It leaves your phone only when you choose to share it, through the share sheet, to a destination you pick.
Children
The app is not directed at children under 13. We do not knowingly collect data from children.
Your rights
Because your data never reaches us, there is nothing held by Patient Thread Ltd to access, correct or delete. You control your records entirely. To delete a single entry, swipe it or use its menu. To delete everything, delete the app. Device backups can be managed in your iCloud or Google account settings.
